If you wish to register as a mason and marble manufacturer/contractor, you may obtain the application form through the following means:

1. In person at the Hong Kong Cemeteries and Crematoria Office of this Department;
2. In person at the Kowloon Cemeteries and Crematoria Office of this Department; or
