Business operators confirmed to voluntarily observe the Practical Guide can submit the following registration form for registration online.

I/We hereby confirm that I/we will voluntarily observe the relevant hygiene and food safety guidelines for handling of meals ready for consumption when providing take-away meals and meal delivery services specified in the subject guideline from the date of signing. I/We agree the FEHD to display the information of the aforesaid licensed/permitted premises on the FEHD's website for public reference, until I/we notify FEHD in writing otherwise. I/We understand that FEHD has the sole right to show or remove such information from FEHD's website at any time without the need to obtaining my/our prior consent.
